XMod will handle required fields and you can also use javascript. Without the required fields the form isn't submitted.
It does take a bit of learning but once you have a handle on it, its a very useful tool.
As an alternative have a look at the dnn-modules 'enhanced feedback' module http://www.dnn-modules.com/Default.aspx?tabid=81. This is pretty simple to use.

I have purchased the Complete Feedback Designer 1.3.6 from SnowCovered for $10 http://www.snowcovered.com/Snowcovered2/Default.aspx?tabid=242&PackageID=4665&search=Complete+Feedback+Designer+1.3.5&pagenumber=0&sortby=&tagid=. It works great, I made my own contact form with custom fields.

Ended up goign with Xmod, thanks Bruce.

Only thing I notice is that the page seems a bit slow to load that has the Xmod form on it compoared to other pages.  I assume the issue is the XML Parsing.

Older versons of Xmod can slow down a page some depending upon what is in the form. Its never really been a problem for me because I have the hardware behind my sites to handle the load. I think this has been improved with the later versions.
