I made a PurchaseOrder provider and a SimpleCheckout provider in about an afternoon. It's quite easy...

1)You will find the source in \DesktopModules\Store\Cart\Providers\GatewayProviders (just copy the sample source to your new folder).
2) Find the old and replace with the new then build your provider project.

Deployment

1) Drop the two controls ProviderNameAdmin.ascx and ProviderNamePayment.ascx into \DesktopModules\Store\Providers\GatewayProviders\ProviderName
2) Move your DotNetNuke.Modules.Store.Cart.ProviderNameProvider.dll into the DotNetNuke bin folder.

Your new functionality should be picked up in the Store admin module automatically.
