Is it possible to upgrad from DNN2.1.2 to the latest version and what about home-made modules?

if not, can i make a link between the tow versions without  requiring to log-in twice?

You should be able to make the upgrade but I would do a few things....

I would first upgrade to the newest version of 3, which I believe is 3.3.7, then upgrade to 4.4.1

For your modules I would be sure to test all of this in a separate environment just to make sure that the upgrade process goes smoothly and to also be sure to back everything up!  But there shouldn't be any reason that you cannot upgrade.
