It would be great to have the ability to generate both an on-screen as well as an email invoice for customers who won't be paying by credit card.  Any plans along these lines?

I agree! I have just installed the module, and have not tested it fully yet, but onscreen invoice and emailoption (confirmation to customer and storeowner) would really be excellent!

I would also like to see this option at some point. I may even have a go at trying to implement it myself if it won't be available soon : )
