I think I resolved the issue (at least for my installation)...

Just for testing, I created another portal on the same installation and checked the html function on it.  It worked perfectly.  This led me to the conclusion that it wasn't a config issue on the site but rather something unique to the original portal.  Having installed several 3rd party modules and AD authentication as well, I expected some kind of conflict or other nightmare.

It turns out to be pretty straightforward...

Previously I made a copy of the DK skin into the portal 0  folder (which caused other issues as reported elsewhere)...but it seems have also broken the Insert function of the HTML editor but this is only a guess. I re-sync'ed the portal folder in the file manager and it appears to fix the problem with the editor. 

I have been unable to duplicate the issue by copying other files into the portal sub-folder so this remains a puzzle (at least for me).

All is working now.
